SUPER16 ALL-STAR PARTICIPANT WAIVER AND RELEASE FORM

By selecting the button and submitting this form, you acknowledge and agree to the following terms, including waiving liability and granting consent for the use of photos and videos for promotional purposes.

1. Assumption of Risk and Waiver of Liability

By submitting this form, I, the participant (or parent/guardian of the participant), acknowledge that participation in Super16 All-Star events, including but not limited to camps, combines, and games, involves inherent risks, including physical injuries, accidents, or unforeseen events.

I voluntarily assume all risks associated with participation and hereby release Super16 All-Stars, Recruit Oklahoma LLC, their officers, directors, employees, agents, and sponsors from any liability, claims, or causes of action, whether arising from negligence or otherwise.

This release applies to all activities organized by Super16 All-Stars and Recruit Oklahoma LLC, including travel to and from events.

2. Medical Treatment Authorization

I authorize Super16 All-Stars and Recruit Oklahoma LLC staff to obtain emergency medical treatment for me (or my child) if necessary during any event. I agree that I am solely responsible for any medical expenses incurred as a result of such treatment.

3. Media Release and Consent

By submitting this form, I grant Super16 All-Stars and Recruit Oklahoma LLC the irrevocable right to use any photographs, videos, or other media captured during events that include me (or my child) for promotional purposes.

This includes, but is not limited to, use in printed materials, websites, social media platforms, and other promotional or advertising efforts. I understand and agree that:

  • No compensation will be provided for the use of these materials.
  • Super16 All-Stars and Recruit Oklahoma LLC retain all rights to the media for future use.

4. Agreement and Acknowledgment

By selecting the button and submitting this form, I confirm that:

  • I have read, understand, and agree to the terms outlined above.
  • I (or my child) am physically fit and able to participate in all activities organized by Super16 All-Stars and Recruit Oklahoma LLC.
  • Submission of this form constitutes my full and voluntary agreement to all terms, including the waiver of liability and consent for media usage.

By selecting and submitting, you release Super16 All-Stars and Recruit Oklahoma LLC from all liability and agree to the terms outlined in this waiver.
